【 要旨 】 慢性腎臓病(CKD)患者における糖尿病性腎症や高齢化などの患者背景の変化に加え,二次性副甲状腺機能亢進症に対する治療技術の進歩により,線維性骨炎などの高回転骨の頻度は低下傾向にある一方,無形成骨などの低回転骨の頻度は増加している.無形成骨では骨折の増加や骨折治癒の遅延のみならず,血管の異所性石灰化などに伴う心血管疾患との関連も報告されており,CKD患者の生命予後の観点からも重要な病態と考えられる.近年,骨形成促進薬であるヒト副甲状腺ホルモン製剤(テリパラチド)が使用可能となったが,CKD患者における低回転骨への治療効果や安全性は十分解明されておらず,今後も低回転骨の予後改善につながる予防や治療法の確立が課題といえる.

[ Summary ] The increasing prevalence of diabetic nephropathy and the increasing elderly population, has led to an increased number of patients with chronic kidney disease (CKD). Recent improvements in treatment of secondary hyperparathyroidism have contributed to preventing high-turnover bone diseases such as that associated with osteitis fibrosa. Low-turnover bone diseases cases, particularly adynamic bone disease, have increased in number. Adynamic bone disease in patients with CKD is an important concern because it may contribute to a potential for increased risk of fractures and cardiovascular disease which are also associated with increased mortality. Recently, once-daily teriparatide administration has become available in Japan to treat osteoporosis in patients at high risk of fractures. However, its efficacy and safety in patient with CKD with low-turnover bone disease remains unclear. Further investigations are necessary to establish optimal strategies for prevention and treatment in those patients to improve clinical outcomes.
