【 要旨 】 | 高齢透析患者の終末期医療において,患者をどのように看取るかが重要な課題となってきている.透析の非導入および中止の現状とその課題について報告した. 日本透析医学会は2014年に「維持血液透析の開始と継続に関する意思決定プロセスについての提言」を発表したが,当院では2008年から患者および家族から事前指示書を取得しており,2014年度は入院死亡患者の95%から事前指示書を取得し看取りを行った.その結果,人工呼吸器装着が減少し,維持透析の中止の症例が増加した.中止から死亡までの期間は平均8.3日であった.患者の意思を尊重した緩和ケアを医療チームとして提供する必要があり,家族に対しては看取り時および看取り後も精神的および社会的な支援を行う必要がある. |

[ Summary ] | Provision of end-stage medical care for elderly patients on maintenance dialysis has become an important issue. We report on the present circumstances and problems involved in withholding or discontinuing maintenance dialysis. In 2014, the Japanese Society for Dialysis Therapy published an opinion on the decision-making process for beginning or continuing maintenance dialysis. Our hospital began obtaining advance directives from patients and their families in 2008. In 2014, 95 % of patients who died had received end-stage care based on advance directives before their deaths. This resulted in fewer patients requiring ventilator support and more patients discontinuing maintenance dialysis. The mean period from discontinuing dialysis to death was 8.3 days. Provision of palliative care as per patients' wishes requires team effort, and psychological and social support should be given to families during and after end-stage care. |
