| 【 要旨 】 | 近年わが国では高齢の透析患者が増加しているが，高齢透析患者では単に透析治療を行う，あるいは単一の疾患を治療するだけでは個々の患者の総合的な改善にはならない場合が多い．なぜなら高齢患者は多くの合併症を有しており個々の疾患が互いに影響し合うこと，認知症や身体機能の低下から在宅生活の維持，透析への通院手段などに問題が生じることが多いこと，介護が必要であっても適当な介護者がいない場合があることなど，高齢透析患者がかかえる問題は多岐にわたる．こういった観点から，疾病だけでなく，身体面，精神心理面，社会面から総合的に生活機能障害を評価する高齢者総合機能評価（comprehensive geriatric assessment；CGA）を用いることが有用である．CGA評価を行う際は，各施設・病院に合った評価方法を用い，その結果を医療スタッフ全員が共有して問題点を認識することが必要である．そして単に評価するだけにとどまらず，CGAで得られた問題点に対して方策を立てることで患者の予後やQOL改善につなげるように取り組むことが重要である． | |

| [ Summary ] | A growing number of elderly patients receive dialysis therapy in Japan. Performing dialysis or treating a single disease dose not often improve the overall condition of elderly patients. Elderly patients experience many problems. Those difficulties, characterized by multi-morbidity conditions, include declines in cognitive ability and daily functions, as well as social ploblems. Because of these factors, it is useful to use CGA to evaluate not only disease but also physical, psychological and social functions. It is important that proper evaluating methods be used in ways which are suitable for individual medical facilities. All medical staff members share both the results from and problems associated with CGA. Countermeasures for problems resulting from CGA may improve the prognoses and quality of life for these patients. |
