【 要旨 】 腎硬化症は，長期にわたる高血圧により生ずる腎病変である．軽度～中等度の高血圧により腎臓の細動脈硬化病変を生じ，これに伴う腎障害を生ずる．これが良性腎硬化症である．近年，透析導入患者の高齢化に伴い，腎硬化症による末期腎不全により透析に至る症例が増加している．このような症例は，慢性糸球体腎炎による腎不全と異なり，全身の細動脈の硬化性病変と大血管の動脈硬化病変を伴っている場合が多い．そこで，透析導入時に臓器合併症の有無を確認，治療をすること，さらに，vascular access作成時には，血管の動脈硬化や血流に配慮して部位を決定することも必要である．また，脳血管疾患や冠動脈疾患，末梢動脈疾患の発症や悪化を予防するために，適切な全身管理がとくに重要である．

[ Summary ] Nephrosclerosis is defined as renal lesions occurring in a setting of mild to moderate hypertension, associated with hyaline arteriolosclerosis. Recently, the average age of patients starting renal replacement therapy has increased. Thus, the number of cases of nephrosclerosis has increased as the cause of end-stage renal disease. These patients sometimes also have vascular complications. We need to treat these patients with special caution in order to prevent the onset of complications.
