【 要旨 】 透析導入患者の高齢化が進み，もっとも割合が高い年齢層は男女ともに75～80歳となっている．高齢者の特徴とPD療法のメリット・デメリットを考慮すると，高齢者にはPDファーストが薦められるが，PD療法の普及率には地域差が大きい．すなわち，透析医療機関，腎臓・透析専門医，そして在宅医療に不可欠な訪問看護ステーションの数的地域差，かつPD対応可能施設の地域差が大きな要因である．PD療法は特別な医療ではなく，必要な情報提供やスタッフ教育で，どこでも可能な医療として全国に普及させることが必要である．

[ Summary ] Dialysis treatment has become prevalent for the elderly,particularly for patients aged 75 years or older. Because of the progress being made in peritoneal dialysis (PD) treatment elderly patients have excellent outcomes even with multiple comorbid conditions. PD is a treatment method which should be considered as the first choice for introducing dialysis in elderly patients. The regional prevalence of PD has a close relation to the numbers of dialysis centers, nephrologists and home-visit nursing care centers. Adequate information and education are necessary for the use of PD in individual areas in Japan.
