【 要旨 】 血液透析患者は食塩摂取がそのまま体重増加に直結するため,その管理はとくに重要である.今回,導入後約1年が経過し,尿量の減少とともに体重増加量が多くなった高齢患者にステップ・バイ・ステップ方式で目標を設定し食塩摂取制限に対する支援を行った.その結果,患者・家族は食塩摂取制限に向けた行動を獲得し,継続することができた.看護実践を振り返ると,神谷が提示した「6ステップモデルを応用した看護展開モデル」に沿った支援であった.一般的に高齢患者の行動変容は困難といわれている.問題の明確化や動機づけは重要であり,モデルに沿った看護支援は高齢患者の自己管理へのアプローチとして有用であった.

[ Summary ] The management of salt intake is particularly important for hemodialysis patients, because dietary salt intake results in interdialytic weight gain. We conducted a step-by-step, goal setting intervention for salt restriction in elderly patients who had received hemodialysis for approximately one year and exhibited increased weight gain along with decreased urine volume. As a result, these patients and their families developed and maintained behavioral changes to help them reduce salt intake. In terms of nursing practice, it was found that this intervention followed the "six-step nursing process model" introduced by Kamiya. Lifestyle changes are believed to be difficult for elderly patients to achieve and maintain. The present study demonstrates that it is important to identify patients' problems and promote their motivation to make changes. This model based nursing intervention is an effective approach to support the self-management of elderly patients.
