【 要旨 】 「住み慣れた土地での生活を支える」というのが，国が示している高齢者対策の一つである．地域での安否確認・見守りがすすめられているが，認知症の徘徊による行方不明や事故死が増加していることがニュースでとりあげられ，さらなる対策が急務である．また，透析医療の現場においても認知症を背景とした治療や通院の拒否行動，透析中の自己抜針，透析後の徘徊等さまざまな問題があり，体外循環という危険と隣り合わせのなか，医療者側はなんとか安全に治療が行えるよう苦慮している．時に薬剤で鎮静をはかり，身体拘束も余儀なくされるのが現状にある．今後さらに増えると思われる高齢透析患者の認知症に対して，私たち看護師は，認知症の正しい知識を身につけその患者にあったケアを提供するとともに，認知症発症の予防に努める必要がある．

[ Summary ] A basic ethical principal for providing medical care for the elderly has been to extend life at all costs. Due to media reports concerning the deaths of patients with dementia, additional safety measures have been recommended. Attention must also be paid to these patients to avoid issues associated with their behavior such as them removing their own needles or post-dialysis loitering. Currently, sedation and physical restraints are the means being employed to control these patients. Prevention and treatment of dementia will become increasingly critical as our population ages.
