【 要旨 】 腎移植件数は年々増加傾向にあり，先行的腎移植を受ける患者も増えている．そのため，末期腎不全の患者が，透析（血液透析・腹膜透析）か腎移植を選択するとき，患者やその家族が正しい情報を十分理解したうえで治療選択が行えるよう意思決定支援が必要である．また，移植が増加する一方で，透析再導入となる患者がいる現実もある．より長く移植腎機能を保つために，移植前から移植後の生活を考えた患者教育も重要である．患者の治療選択時，透析再導入時には，身近な存在であるレシピエント移植コーディネーターと透析スタッフが患者に寄り添い，その人らしい意思決定が行えるよう連携していく必要がある．

[ Summary ] The number of renal transplants is increasing yearly. In addition, the number of pre-emptive renal transplantations is also increasing. Therefore, it is important to support decision-making by patients and their families when choosing medical treatments for end-stage renal failure. While the number of renal transplants is increasing, there is also the reality of recipients who must have become dialysis again. In order to maintain graft function longer, patients must receive pre-transplant education concerning daily living. In order for patients to make correct decisions, recipient transplant coordinators (RTC) must cooperate with the dialysis staff when these patients choose medical treatments for end-stage renal failure and dialysis re-introduction.
