【 要旨 】 生体腎移植施行に当たってはドナーの安全性が最優先されねばならない．当施設では1970年より生体腎提供の条件を，(1) 自発的な提供の意思があること，(2) 活動性の感染症，悪性腫瘍などがないこと，(3) コントロール不良な高血圧，糖尿病，代謝異常などがないこと，(4) 腎に関する条件として，尿蛋白陰性，CCrが80 mL/min以上，両腎の機能に大きな差がない，解剖学的な著しい異常がないものとして生体腎移植を行ってきた．今回の検討において長期予後においても一般人口と比して劣るものではなく，これまでのドナー選択，手術手技，術中術後管理に大きな問題はなかったと考えられる．しかし，今後も臓器不足や生活習慣病の増加に伴い生体腎ドナーも糖尿病，高血圧などを合併したマージナルドナーが増加拡大していく可能性も考えられる．この点で腎臓内科医の協力の下に年に1～2回の外来フォローを含めた長期の定期的フォローが必要である．

[ Summary ] The indications for living-donor kidney donation have expanded in terms of donor medical status as well as human leukocyte antigen matching, and ABO blood type (ABO) incompatibility, As a result, emphasizing donor shortages is necessary. Because hypertension and diabetes may impair the function of the remaining single kidney after donation, living kidney donors need to be selected carefully.
Our retrospective investigation revealed that the short- and long-term outcome for previous kidney donors were satisfactorily accepted.
