| 【 要旨 】 | オンラインHDFでの除去効率は置換液量に依存しているが，希釈方式により効率が異なる．置換液量設定は血流量，透析液流量，ヘモダイアフィルタの透水性により規定され，蛋白漏出型ヘモダイアフィルタを使用する場合，後希釈ではアルブミン漏出量が過大となるため前希釈HDFが選択される． 平成24年度の診療報酬改定によりオンラインHDFの技術料が新設され，適応の縛りもなくなった．さらに施設要件として「透析液水質確保加算2 20点」を算定していることが加わった．この改定により，必要とされるすべての透析患者にオンラインHDFを行うことができるようになった．今後，オンラインHDFの有効性に関するエビデンスの構築が必要となる． |

| [ Summary ] | The removal efficiency of online HDF depends on convective volume, prescribed dilution methods, blood flow, dialysate flow, and the water permeability into hemodiafilters. When a protein-permeable dialyzer was used for post-dilution HDF, albumin leakage rapidly increased. Pre-dilution HDF is often selected as a treatment procedure. Because of revisions in the reimbursement system for medical services made in 2012, a technical fee for online HDF was established and "the restrictions concerning indication" for treatment have been eliminated. In terms of facility requirements, calculations of "additional parameters to maintain dialysis fluid water quality" were added also. Since this amendment was implemented, we have been able to perform online HDF on all dialysis patients. In the future, it will be necessary to accumulate evidence on the effectiveness of online HDF. |
