【 要旨 】 維持透析患者でよくみられる上部消化管疾患として，出血性胃病変，胃・十二指腸潰瘍，血管性病変（GAVE），下部消化管疾患として，大腸癌，虚血性腸炎，アミロイドーシスについて，診断と治療の立場から述べた．加えて，近年，内視鏡的に到達が困難であった小腸病変発見に一石を投じたカプセル内視鏡並びにシングルバルーン法の併用により見つけられた病変の診断と治療，および高齢で心血管系合併症が多く出血傾向を有する早期胃癌の治療法の一つとしてICG（indocyanine green）併用半導体レーザー照射法についても言及した．

[ Summary ] Upper and lower gastrointestinal tract diseases, including hemorrhagic gastric mucosal lesions, gastroduodenal ulcers, gastric cancer, gastric antral vascular ectasia (GAVE), colon cancer, ischemic colitis and amyloidosis are often observed in patients receiving maintenance dialysis. The diagnosis and treatment of these diseases, are summarized, with special reference to capsule endoscopy and single balloon enteroscopyidentified intestinal lesions. These have long been a challenge to the endoscopists. We also assess enhanced diode laser ablation using ICG submucosal injections as a promising treatment for early gastric cancer in elderly patients with cardiovascular and hemorrhagic complications.
