CKD-MBD is commonly observed in the dialysis and pre-dialysis periods. It is often underestimated in kidney transplant recipients (KTRs). The pathophysiology of this condition is complicated and many times starts before transplantation. However, it is characterized by drastic changes in laboratory parameters soon after transplantation and uncoupling of bone mineral metabolism induced following immunosuppressive therapy and progressive renal dysfunction. Because we do not have any effective solution for treatment during the post-transplant period, the legacy effect of CKD-MBD gained during the dialysis period is rather important. Thus, long-term management of CKD-MBD is essential. In this section, we first introduce the pathophysiology of CKD-MBD in KTRs along with the new Japanese guideline for CKD-MBD in KTRs. We then reflect on what health care providers should take into account in the course of transplantation. |
