【 要旨 】 腎臓の機能は老廃物の排泄，水，電解質，酸塩基のコントロールにとどまらず，レニン分泌，エリスロポエチンの産生やビタミンDの活性化などホルモン産生代謝にも関与するなど多岐にわたる．加齢に伴い，腎臓は組織学的・生理学的な変化を生じ，その結果，若年者とは異なる体内環境となり，いろいろな臨床像に関与する．そういった高齢者の腎機能を正確に評価するためには，筋肉量に左右される血清クレアチニン値を指標とするより，近年提唱された推定糸球体濾過量の計算式を用いることが望ましい．また，血清シスタチンC値は今後の検討を要するが，早期腎機能障害を検出するマーカーとして期待される．

[ Summary ] The kidneys play an important role in water and electrolyte balance and acid excretion and hormone secretion. Structural and physiological changes are found in kidneys of the elderly, leading to a decrease in renal functions. The Japanese equation for estimated GFR is convenient and useful to evaluate kidney function in older adults.
