【 要旨 】 透析導入患者と維持透析患者の高齢化は年々著明となり，多くの合併症を抱える高齢透析患者における介護は一般高齢者以上に深刻な問題となっている．わが国の透析人口全体の9.8％が認知症透析患者であり，その50％にサポートが必要であることから，適切な要介護認定と患者のニーズに応じた支援が不可欠である．要介護認定の実際と，主治医意見書の役割，透析患者における介護保険の活用について述べた．

[ Summary ] More than half the patients starting dialysis now are over the age of 65 and the proportion of dialysis patients aged over 75 years is increasing. 9.8 % of the total dialysis population have dementia in Japan. There are specific problems for dialysis patients with dementia. The existing nursing care insurance (long-term care insurance) system plays an important role in supporting dialysis patients.
