【 要旨 】 腎不全医療には，慢性腎臓病（CKD）から透析導入，腎移植に至るまでとその後の管理，時には急性腎障害の対応も含めた，包括的な観点から捉える必要がある．腎不全の早期発見，早期介入のためには，地区自治体や地域の医療機関との連携が不可欠である．かかりつけ医と腎臓専門医の連携を促進するための診療システムの有用性を検討する，腎臓病重症化予防のための戦略研究が現在進行中である．
CKDのさまざまなステージにおいて，地域連携が成果を上げるためには，医師，看護師をはじめ医療スタッフ全体が携わることのできる，情報共有ツールや体制づくりが望まれる．

[ Summary ] Health care for renal disease includes care for chronic kidney disease (CKD), renal replacement therapy and treatment for acute kidney injury. In order to provide early detection of and intervention for CKD, a cooperative health care among community network is necessary. A strategic outcome study for chronic kidney disease termed the "Frontier of Renal Outcome Modifications in Japan". was designed to encourage CKD patients to consult their physicians, enhance cooperation between nephrologists and general practitioners, and prevent the progression of kidney disease. A CKD care system which will promote collaboration between medical professionals must be established.
