【 要旨 】 現在30万人近い腎代替療法を必要とされる人々がいて，実に97％近い人々が施設血液透析を受けている．なぜほとんどの人々が施設透析を受けているのか．それは施設透析側が多大な努力を払っている結果と考えられる．一方，透析を受ける側は高齢化と糖尿病腎症で合併症を有する人々が増加している．さらに，家庭で病人を看るということは多くの地域で困難となっている．このような複雑な総合的要因が在宅透析が伸びないことにがっていると考えられる．それをどう打破するのか．それには透析を行う側，および受ける側のより積極的努力と，さらに施設側の経済性をやや無視するといった姿勢が重要であると考えている．

[ Summary ] In Japan, approximately 300,000 people receive dialysis therapy and 97 % of these patients do so at hemodialysis centers. This trend is probably due to the tremendous efforts made to develop dialysis centers. Many patients receiving hemodialysis are elderly and diabetic and have a great number of complications. In relation to these factors, it is not easy to provide care for patients in their homes. To overcome these complications, additional effort is required by patients as well as providers. It is absolutely necessary to develop strategies to advance quality of dialysis care.
