【 要旨 】 必至の不治の状態で死が迫っている終末期維持透析患者の医療とケアにおいては,① 病態(余命)の把握と推測,② 患者・家族への告知(真実の語り),③ 患者・家族の理解と納得の確保,④ 患者・家族への寄り添い,⑤ 患者希望(意向)の尊重,⑥ 何をどこでだれが行うかのプラン作成(苦痛・苦悩の軽減),⑦ 看取り(命終焉)の場の選定,⑧ 患者死後の家族のグリーフケア,⑨ 医療者の終末期関連の知識と実技などの習得が緊急の課題となる.いずれの判断と決定にも,逡巡と心理的な負担を伴う難業である.ことに患者に終末期を告げることや希望を聴取することには,医療者としてためらいと迷いが払拭できず,思わず腰が引けてしまう.これを乗り越えなければ,充実した終末期の医療とケアは望めない.

[ Summary ] Urgent problems confronted in therapy for and care of terminal stage maintenance dialysis patients who are untreatable and facing death include, 1) disclosure of condition (remaining lifespan) and prognosis, 2) notification (telling the truth) to the patient and family, 3) securing the understanding and acceptance of the patient and family, 4) developing a closer relationship to the patient and their family, 5) respecting the wishes of the patient, 6) making a plan for what measures will be performed by whom and when they will be performed (reduction of pain and suffering), 7) choice of where nursing care (end of life) will take place, 8) grief counselling for the family following death of the patient. Whichever decisions are made, it is a difficult task, often accompanied by anxiety and psychological pressure. As medical professionals, our hesitation and doubt can never be completely removed causing us to instinctively refrain from making decisions, especially when informing patients about their terminal state or when questioning them about their wishes. If this hesitation cannot be overcome, there is no hope of providing appropriate medical treatment or care for patients' terminal stage care.
