【 要旨 】 限外濾過率とふるい係数(SC)は,濾過の効率および分離能を規定する2大因子である.後希釈法の血液濾過,血液透析濾過の臨床では限外濾過流量(QF)は,濾過器内の濃縮のため血流量の1/3程度を限度に設定する.ポリスルホン系の膜では,アルブミンのSCが初期において高値を示すので,アルブミンの漏出が懸念される.ただしこの現象には,ポリビニルピロリドンのような親水化剤が関与している可能性が高く,すべての膜について当てはまる一般的な事実ではない.QFの増加に伴い低分子蛋白質のSCは低下するので,QF×SCの増加は緩慢となる.

[ Summary ] The ultrafiltration rate (UFR) per pressure and the sieving coefficient (SC) are two important parameters in ultrafiltration. The ultrafiltration rate (QF) in post-dilution hemofiltration and/or hemodiafiltration is usually set equal to 1/3 of the blood flow rate at most due to the concentration in the hemofilter. The SC for albumin in polysulfone membranes generally takes the maximum at the beginning of the experiment, which suggests that a large amount of albumin loss may be observed at the beginning of treatment. This phenomenon, however, is possibly caused by the amount of hydrophilic agents such as polyvinylpyrrolidone and cannot be accepted as a general rule that applies to all commercial dialysis membranes. Since SC for low molecular weight proteins decreases with the increase of QF, the product of QF and SC slowly increases.
