[ Summary ] |
The ultimate goal of care for chronic kidney disease is to improve patient quality of life and longterm outcome. Life style modification and adherence to medication regimens necessitate the patients' understanding of the disease and its treatment. We have developed and opened a multidisciplinary chronic kidney disease clinic comprised of nephrological care, one on one nursing education, and a group education program known as the "Soramame Juku" or "Kidney Bean School". In developing our group education program, we adopted a logic model and implemented use of adult learning theory. We briefly describe the process of developing and implementing our group education program. |
