【 要旨 】 透析患者の終末期に際し，もっとも患者のそばにいる病棟医は，患者の予後，透析継続の是非を医学的に見極めたうえで，患者本人・家族と十分に話し合い，透析延期・中止も含めた医療方針を決めていく必要がある．米国で使用されている“Shared decision-making in the appropriate initiation of and withdrawal from dialysis”は，これらの方針を決めるうえでの一つの指針となるが，本邦の現状・法律に即していない難点をもつ．今後，日本独自の終末期透析患者の治療に対するガイドラインと，医療者に対する法的保護の整備が望まれる．

[ Summary ] Palliative care is one of the most integral components of patient management for the nephrologists. When we observe patients in terminal states, we must formulate their prognoses and judge theirdialysis needs. The guideline termed "Shared decision-making in the appropriate initiation of and withdrawal from dialysis" is a tool for decisionmaking related to end of life care. Cultural norms and personal opinions must be considered when establishing end of life protocols for Japanese patients.
