【 要旨 】 透析患者では慢性腎不全，透析療法に関連したさまざまな病態，合併症の対応が課題である．尿毒素物質の蓄積や内分泌異常による腎性貧血，二次性副甲状腺機能亢進症，透析アミロイドーシスなど透析患者に特有な疾患のほか，心血管障害など健常人と比較して発症頻度の高い疾患など多岐にわたる．chronic kidney disease-mineral and bone disorder (CKD-MBD)，cardio-renal anemia (CRA) 症候群 (心腎貧血症候群)，malnutrition-inflammation-atherosclerosis (MIA) 症候群など近年提唱されている疾患概念は，既知の病態との関連を明らかにし，透析患者に頻度の多い心血管障害，動脈硬化などの病態の解明に迫るものである．これらは最終的には透析患者の生命予後，ADLあるいはQOLの改善につながるものであり，病態の解明のみならず治療法および予防法の確立が期待される．

[ Summary ] Uremia in hemodialysis patients is related to various types of general disorders, such as anemia, secondary hyperparathyroidism, and dialysis-related amyloidosis. Recently, some systemic disorders, such as chronic kidney disease-mineral and bone disorders (CKD-MBD), cardio-renal anemia (CRA) syndrome, malnutrition-inflammation-atherosclerosis (MIA) syndrome have been suggested as causative factors in dialysis patients having more cardiovascular events and worse prognoses than other patients. Establishing more effective disease preventative measures and dialysis therapy techniques will improve not only mortality rates but also overall quality of life for patients in the future.
