| 【 要旨 】 | 高齢慢性腎臓病患者は，腎機能の悪化だけにとどまらず，腎機能と加齢に伴う心血管病変を主徴とする多彩な合併症，家族関係の変化，療養生活を継続していく困難さ，介護など多様で複雑な問題を抱えている．そうした問題のなかで腎代替療法を受けるか否かの決断も大きな課題である．看護師は，高齢慢性腎臓病患者一人ひとりの今までの生活史や日々の生活の変化を知り，病状の進展，加齢によって変化する日常生活を予測し，患者だけでなくその家族にとっても最良の方向を見出していくことが重要である．そのためには，高齢慢性腎臓病患者への外来受診時から早期の専門的な看護介入と，看護介入が有効に機能するシステムの構築が必要であると考える． | |

| [ Summary ] | Elderly patients with chronic kidney disease (CKD) complications experience more serious problems than younger patients. This is true not only for physical conditions, including cardiovascular diseases, progressive kidney dysfunction and others but also for emotional and social difficulties. It is important that nursing staffs should know the natural history of patients well and understand the change in their lives produced by CKD for better control of elderly CKD patients' conditions. Nursing staff members should be able to accurately predict deteriorations in patients' daily lives according to their age and the progression of CKD and its complications. Staff members should find the best ways for patients as well as their families to solve health related problems. Early intervention by Certified Nurse Specialists is essential to maintain the well-being of elderly CKD patients longer. This intervention system should be developed in major hospitals which manage CKD patients. |
