| 【 要旨 】 | 在宅透析が，今，必要な理由は，施設透析では健保上の規制により質のよい透析医療が維持できなくなることと，5年後からは減少するであろう透析者を，透析施設や医療スタッフを増やすことなく対応しなければならない医療環境上の事情による．在宅透析の適応者は「やってみたい」と考えるすべての透析者であるが，現状では身内の介助者の有無や自己穿刺が可能か否かが必要条件としてあげられている．在宅透析の透析効率は透析施行条件のすべてが透析者自身に合わせられることから，透析時間や施行回数を含めて十分な透析量が確保できることからも施設透析よりは有利である．そのことが生命予後にもよい影響を及ぼしている． | |

| [ Summary ] | The reason home hemodialysis (HHD) will become necessary in the near future is that circumstances concerning the medical environment must be adjusted to the decreasing dialysis patient load in the next 5 years. Increases in the number of dialysis facilities and medical staff can not be maintained along with high-quality dialysis therapy, due to regulations by the National Health Insurance, Program, concerning dialysis facilities. HHD patients hoping for continued HHD must adapt. Presently HHD is determined to be feasible when there is a familial helper and when self centesis is possible. Home hemodialysis is a self managed dialysis therapy for end-stage chronic renal failure patients. Therefore, dialysis efficiency is higher than that seen with in facility dialysis. As a result, vital prognoses also improve, because the QOL is increased in patients receiving home hemodialysis. |
