| 【 要旨 】 | 透析は自己管理と並行する治療であり，患者が知識を得て理解を促進することや，ライフスタイルを透析療法と融合させるために変化させ，治療に必要な医療上の指示を守れるようにその方法を模索することが必要である．ノンコンプライアンスという言葉は，「指示を遵守できない」という意味をもち，医療者側からみた患者評価となりやすい．そこで，慢性疾患患者の自己管理援助として，患者の生活の視点から実施の困難さや障害を明らかにすることが必要である．患者が自分自身を支えるために自分で責任をもち，役割を遂行するという，アドヒアランスという考え方で，支援していくことが重要である． | |

| [ Summary ] | Dialysis is a form of treatment that works in parallel with self-management. It is necessary for nurses to grasp treatment strategies to help patients acquire knowledge, increase understanding and modify their life styles in line with dialysis therapy, needs so that they can comply with medical instructions. Non-compliance means patients can not follow the instructions, and it is a term that should be understood hy health care providers. Therefore, it is necessary to clarify the difficulties patients face in carrying out self-management instructions to provide support for chronic dialysis patients. It is important to support our patients based on the concepts of adherence, which means that our patients can support themselves by taking responsibility and accomplishing their role in self care management. |
