【 要旨 】 高齢者や糖尿病性腎症による透析導入の増加や長期透析患者の高齢化に伴い，循環器合併症は透析患者の死因の約半数を占め，生命予後を左右する重要な病態である．とくに，透析中の血圧の変動や不整脈は，透析導入に至った原疾患のみならず，体外循環および循環血液量減少の影響が大きい．安定透析をするためには，透析方法の工夫が必須である．とくに透析間の体重増加のコントロールが重要であり，根気よく再指導を行い，セルフケアに対する患者のモチベーションを上げる努力が必要である．

[ Summary ] Recently, the number of elderly patients and diabetic patients starting hemodialysis has increased. The average age of long term hemodialysis patients has also increased. In these circumstances, cardiovascular complications account for over half of the total number of hemodialysis patient deaths. Changes in blood pressure and the occurrence of arrhythmia are influenced by the original ESRD condition, extracorporeal circulation, and decreases in circulating blood volume. To conduct hemodialysis sessions safely, it is necessary for various factors concerning hemodialysis to be observed. The most important factor for treatment is control of inter-dialytic body weight gains. To achieve this, medical staff members should patiently provide motivation to hemodialysis patients on a repeated basis.
