【 要旨 】 維持血液透析 (HD) 患者における生命予後と血圧の関連には多くの議論がある.本稿ではHD患者におけるイベント指標としての血圧値,HD患者全般を対象とした,また心不全,虚血性心疾患,脈圧の増大など特定の合併症や病態を有するHD患者を対象とした観察研究 (コホート研究) など,これまでに発表された血圧および生命予後と血圧に関する最近の報告を概説し,あわせてそれらを基にしたHD患者における至適血圧レベルと血圧管理目標について述べた.

Title Blood pressure and survival rates in chronic hemodialysis patients

[ Summary ] Blood pressure is an important issue in the management of patients undergoing maintenance hemodialysis. However, the relationship between blood pressure and survival is still controversial in chronic hemodialysis patients. The importance of and the differences in various blood pressure levels as indices for the diagnosis of hypertension in chronic hemodialysis patients are discussed. An attempt was also made to reconcile the conclusions obtained from recent observational cohort studies. These studies were made using chronic hemodialysis patients, including patients with complications such as congestive heart failure, ischemic heart disease or high pulse pressure. Although defining an ideal blood pressure goal is not possible in the absence of randomized control studies, it is suggested that predialysis and postdialysis goal blood pressure should be approximately 140 / 90 mmHg and 130 / 80 mmHg respectively to achieve improvements in survival rates for chronic hemodialysis patients.
