Blood pressure is an important issue in the management of patients undergoing maintenance hemodialysis. However, the relationship between blood pressure and survival is still controversial in chronic hemodialysis patients. The importance of and the differences in various blood pressure levels as indices for the diagnosis of hypertension in chronic hemodialysis patients are discussed. An attempt was also made to reconcile the conclusions obtained from recent observational cohort studies. These studies were made using chronic hemodialysis patients, including patients with complications such as congestive heart failure, ischemic heart disease or high pulse pressure. Although defining an ideal blood pressure goal is not possible in the absence of randomized control studies, it is suggested that predialysis and postdialysis goal blood pressure should be approximately 140 / 90 mmHg and 130 / 80 mmHg respectively to achieve improvements in survival rates for chronic hemodialysis patients. |
