【 要旨 】 透析患者の消化管悪性腫瘍に対する外科手術は,以前より安全に施行できるようになってきている.しかし,患者の高齢化や透析期間の長期化,糖尿病患者の増加に伴い,術後合併症から不幸な転帰を遂げる症例も存在し,依然として周術期管理には十分に慎重を期す必要がある.手術成績から他病死を除いた成績では,腎機能正常者の手術成績と比較して十分に満足できる結果であった.透析患者の消化管悪性腫瘍の手術では,常に透析そのものの予後を考慮する必要がある.根治性を追及しつつ,周術期の安全性を考慮に入れた術式の選択が重要である.今後は内視鏡手術や腹腔鏡手術などさらに低侵襲な術式の普及が望まれる.

[ Summary ] Surgery for malignant tumors in the digestive organs has widely been performed safely even in patients who are receiving maintenance dialysis. However, it is thought that dialysis patients are still in a high risk group because of increases in the number of elderly patients, as well as diabetic patients, and the long duration of dialysis therapy. Although overall mortality rates were high, the survival rates in dialysis patients with stomach and colon cancer were comparable with the general population, when other causes of death (e. g. cardiovascular disease), except for death due to cancer, were excluded. In this study, the majority of the causes of death after surgery for stomach and colon cancer were not related to the cancer. High morbidity and mortality rates in dialysis patients were responsible for the increased incidence of complications and deaths after surgery. Better outcomes after surgical treatment for dialysis patients with cancer rely on meticulous and intensive perioperative care, in addition to positive surveillance and early diagnosis.
