The prevalence of obesity and metabolic syndrome (Met S) is increasing worldwide. Subjects with Met S are often afflicted with chronic kidney disease (CKD). Therefore, they may develop end-stage renal disease (ESRD). Recently, it has been shown that the incidence of cardiovascular diseases is higher than that of ESRD among subjects with Met S. The mean BMI of incident dialysis patients is increasing in the USA, being over 27 kg / m2 at this time. On the other hand, it is still lower than 21 kg / m2 in Japan. According to registry data from the Japanese Society for Dialysis Therapy (JSDT) the prevalence of diabetic dialysis patients has increased linearly. Diabetes has been the leading cause of ESRD since 1998 and is observed in over 40 % of incident dialysis patients. Precise data on the prevalence of Met S are not yet available from the JSDT. Subjects with Met S may develop non-diabetic CKD and ESRD. Therefore, it is possible that Met S is also increasing in the Japanese dialysis population. Recognition of Met S is necessary, but its clinical significance remains to be determined in the dialysis population. |
