【 要旨 】 生体内の代謝過程に関与する微量元素やビタミンは重要な役割を果たしている.保存期腎不全患者における微量元素やビタミンを検討する場合には,「欠乏」「蓄積」の2点の異常が生じる可能性があることをまず理解すべきである.とくに,亜鉛やビタミンB6の欠乏状態は,尿毒症と類似の臨床症状を呈するため,注意が必要である.また,近年の健康意識の高まりから,医師が処方する薬剤以外のいわゆる健康補助食品 (サプリメント) を患者が自ら購入し服用している場合も多い.腎不全は,微量元素やビタミンの薬理排泄動態に大きく影響を与えることから,サプリメントの種類やその服用状況によっては思わぬ弊害を招く危険もある.腎不全患者の診療に当たっては,今後はサプリメントの服用状況にも十分な注意が払われるべきである.

[ Summary ] Trace elements and vitamins play various important roles in normal individuals. We should recognize the fact that renal failure may be accompanied by two diametrically opposing abnormalities, i.e., an "accumulation" or a "deficiency" of these substances, as far as the pharmacological dynamics are concerned in patients with chronic kidney disease (CKD). In particular, zinc and/or pyridoxine deficiency in CKD patients are clinically important because they may occasionally present symptoms similar to those of uremia, and may even confuse nephrologists whether as to symptoms are due to uremia, or zinc or pyridoxine deficiency.
Additionally, there is an increasing number of patients who have been ingesting various health supplements besides the prescribed drugs. Renal dysfunction has a great influence on the status of trace elements and vitamins in terms of pharmacological effects. Therefore, there is a risk of unexpected accumulations of trace elements and/or vitamins from these supplements in CKD patients. In the future, nephrologists should pay more attention to the use of supplement intake in CKD patients.
