【 要旨 】 血液浄化法におけるラジカル関連マーカーの動きについて血液透析 (HD) と血液濾過透析 (HDF) で比較した.4種の血液浄化法 (HD,on-line HDF: pre dilution,on-line HDF: post dilution,push/pull HDF) を施行し,ラジカル関連マーカー:pentosidine (total,free),CML (total,free),およびケミルミネッセンス法にて,hydroperoxid radicalの変化をみた.
ラジカル関連マーカーであるtotal-CML,total-pentosidineともに,HDでは増加したがHDFでは減少を示した.HDと比較してHDF (post dilution,pre dilution) ではhydroperoxid post/pre ratioは有意に低かった.
HDと比較してHDFにては,(1) 蛋白喪失による蛋白結合CML,pentosidineの直接的除去と,(2) ラジカル担体蛋白,補体蛋白等の除去と希釈効果によるラジカルストレス減少とそれに伴うCML,pentosidineの合成抑制が推測された.

Title The change of radical related markers in several blood purification modalities

[ Summary ] We evaluated patients treated with HDF and HD in view of radical stresses encountered in the course of blood purification.
Chronic dialysis patients were randomly subjected to four types of blood purification treatment. Determinations were made for radical related markers in relation to: total-pentosidine, free-pentosidine, total-CML, and free-CML. In addition, chemiluminescence was used to observe changes in plasma radicals.
Total-CML and total-pentosidine increased when the patients were treated with HD, but decreased significantly in comparison with HDF, they were treated with HDF. Post dilution and pre dilution with HD revealing that the hydroperoxide post/pre-dilution ratio was significantly lower.
The reason for the decrease in AGEs in the HDF is considered, first, to be because of the direct removal of CML, pentosidine (radical related markers) through the loss of proteins.
The second, consideration was the inhibition of the promotion of CML, pentosidine (radical related markers) with a decrease in radical stress during blood purification by both removal of radical carrier proteins or complement proteins such as factor D, as well as the dilution of plasma with massive substitution of fluids, was observed.
