【 要旨 】 わが国においても診断群分類 (diagnostic procedure combination ; DPC) による包括医療が2003年より特定機能病院で導入された．DPCの概要を解説するとともに，大学病院における包括医療の功罪を考察した．とくに血液浄化療法における包括医療にはいろいろな問題点があり，今後医療の質を維持しながら患者にとって満足がいく包括医療となるように必要な見直しが行われることが望まれる．

There are various problems with comprehensive medical care in the field of blood purification, therefore we must reconsider DPC to maintain high quality of medical care to satisfy patient's requests.
