| 【 要旨 】 | 透析室開設から3年を経て，透析室看護師による保存期の生活指導や治療法の選択，導入までの指導体制の構築により，当院の透析導入の21％が腹膜透析を選択し，患者や介護者の望む家庭生活が営まれている．地域における腹膜透析の支援が初めての状況からスタートし，医師会や訪問看護ステーションに向け学習会やスタッフミーティングを通し腎不全の治療と看護を広く理解してもらうための取り組みを進めてきた．地域の最前線で市民の健康を支えているスタッフが，実感をもって腹膜透析のよき理解者となりうるために，公立病院の看護師が自ら腎不全看護の情報発信源となる役割が果せると考える． | |

| [ Summary ] | A system of education and patient care in patients undergoing CAPD in community-based hospital was introduced. CAPD was selected in 21% of ESRD-patient for the dialysis modality. This rate is extremely high comparing an average rate of CAPD in an annual report of total dialysis population by JSDT. The key of success is provided by not only an intensive education and care system in the hospital but also total cooperated support by a visiting nurse, a care manager and a CAPD nurse in the hospital after returning to home.
