【 要旨 】 SMAP法 (stepwise initiation of peritoneal dialysis using Moncrief and Popovich technique) は段階的に手術を行い,腹膜透析に導入する方法である.まず,腹膜カテーテルを腹腔内に挿入後,出口を作らず皮下に埋没し,カテーテルと皮下組織との癒合を早く完成させ,その後に埋没カテーテルを引き出し,出口を作製しCAPDを開始する.手術回数が多い,カテーテル埋没中に腹腔内の情報が得られない,本法に即した診療報酬の設定がないなどの問題点はある.しかしカテーテル感染の軽減,十分な透析量が短期間に得られる,手技練習など教育プログラムに専念しやすい,透析に対する受容が得られやすい,迅速に導入ができる,入院期間の短縮,医療費の削減,計画的な透析導入が可能であるなどさまざまな利点があり,急速に症例数は増え,本邦での腹膜透析導入法のスタンダードになりつつある.

[ Summary ] SMAP (stepwise initiation of peritoneal dialysis using Moncrief and Popovich technique) is a new method for the initiation of CAPD. After the implantation of a peritoneal catheter into the abdominal cavity, the external segment of the catheter is completely buried and remains in the subcutaneous tunnel until exteriorized after several weeks (or months) post-catheter insertion. When an uninfected fixation of the subcutaneous tissue is obtained, the buried subcutaneous catheter is exteriorized and PD is commenced. The disadvantages are an increase in operation frequency, no information about the abdominal cavity while the catheter is embedded and insufficient access to medical insurance payment for this method. However, there are many advantages to this stepwise operative technique, such as a reduction in catheter infection, achieving optimal dialysis dose within a short period, a higher level of concentration for training, an increase in the acceptance of PD treatment, commencement of PD treatment at the proper point, shortening of the hospitalization stays, reduction of medical costs, and better planned initiation of PD therapy. The number of patients employing this method has increased rapidly, and it has become the standard method for the initiation of peritoneal dialysis.
