In order to determine the optimal operating conditions for hemodialysis patients, dialysis dose should be precisely measured by urea kinetic modeling. Several evidence-based clinical practice guidelines for hemodialysis adequacy have been reported in the United States. In cases where we will apply these guidelines to Japanese patients, however, we should pay attention to differences between U.S. and Japanese patients such as body weight, blood flow rates, dialysis time, etc. While extensive effort has gone into guideline development, it is essential to emphasize that guidelines are not standard. Optimal dialysis dose for each patient should be determined by three factors, research evidence, clinical expertise in the affiliation and patient values. |
