【 要旨 】 患者情報を医療チームで共有できない原因は，医療情報の散在，統一性のない記録様式，口頭による共有方法にあった．そこで当院では，多職種が一人の患者に対しての問題リスト，患者計画，経過記録を同一紙面に合同記録する取り組みを行った．合同記録導入に向けては多職種間で話し合い，ルールを決めた．その結果，情報の共有化が進み医療チームとして有効な連携がとれるようになった．

[ Summary ] The need for patient information to be shared came about because it had previously only been transmitted verbally. In our hospital, measures were taken to record patient care plans and progress records on the same page. We discussed many occupational descriptions and decided on rules for joint record keeping introduction. Consequently, information sharing systems have been developed, allowing effective cooperation within medical teams.
