【 要旨 】 透析患者の低血圧には常時低血圧と透析中に生じる低血圧(IDH)があり,IDHは透析患者の主要な合併症の一つである.危険因子には糖尿病,左室肥大,虚血性心疾患,高齢者,透析間の過剰体重増加例などがある.IDHをきたすと限外濾過率の低下,血流量の低下などにより対処することが多く,時には透析時間も短縮されるため,透析の質に影響を及ぼす.その病態としては,(1) 循環血液量の低下,(2) 自律神経機能障害,(3) 末梢血管抵抗の低下,静脈プールの拡大,(4) 心機能の低下,(5) カルニチンの欠乏,(6) 血管作動物質の影響,(7) 降圧薬の影響などがある.なかでも自律神経障害の存在下に血管作動性物質が関与する血管拡張は急激な低血圧をもたらす要因となる.

[ Summary ] Hypotension in chronic dialysis patients is consistent with sustained hypotension and intradialytic hypotension (IDH). IDH is one major complication in hemodialysis patients. Several factors are candidates for being the causes of this condition. They are : diabetes mellitus, left ventricular hypertrophy, ishchemic heart disease, advanced aged, and excessive body weight gain, etc. IDH is a condition which affects the quality of dialysis sessions because of inadequate ultrafiltration volume, reduced blood flow and/or shortened therapy time. Pathophysiological factors related to IDH are summarized as follows: (1) rapid decrease of circulation volume, (2) autonomic nerve dysfunction, (3) absence of decreased venous capacity and/or increased vascular resistance, (4) impaired cardiac function, (5) influence of plasma vasoactive hormones, and (6) effect of anti-hypertensive agents. Vasoactive hormones, especially affect autonomic dysfunction and may induce the sudden onset of hypotension.
