【 要旨 】 日常の透析医療において透析認定看護師に求められる像を医師の立場から述べる.
求められる知識と技量のレベルは,日本透析医学会認定医,透析技術認定士 (臨床工学技士),栄養士,薬剤師,ソーシャルケースワーカーと問題点を共有することができ,その対策についての短期および長期治療計画の立案・実行に主体的役割を果たせることである.また日々の作業のなかから臨床研究のテーマの立案,具体的なプランの作成ができることが望ましい.
Theme Necessity of Expert Nurses in Dialysis Therapy

[ Summary ] The basic knowledge and techniques that an expert nurse should provide in the management and care of patients with renal failure are reviewed. Those requirements are not only limited to dialysis techniques but are also integrated with a wide variety of management tools for ESRD patients, including those in the pre-dialysis stage, the initiation-stage of renal replacement therapy, maintenance dialysis, as well as for hemodialysis, peritoneal dialysis and transplantation.
Generally the responsibility of nurses is limited to carrying out orders from nephrologists and sometimes transmitting patient complaints. A true expert nurse specializing in renal failure-management should have the capability to discover unusual patients events promptly, to think, to judge, to make recommendations to patients, to make suggestions to other staff members and to respond promptly to consultations with patients.
Concrete items to provide nurses who have expertise in renal failure-management are described.
