| 【 要旨 】 | 日本では，一般には，透析患者のような慢性疾患患者の性的問題は軽視されている．患者は性的問題に悩んでいても，医師に積極的には訴えてこない．また，医師も患者の性的問題については質問しない．たとえ患者が性的問題を訴えても，適切に患者の性的問題に対応できる医療スタッフは非常に少ない．医療スタッフは患者の性的問題に多くの関心をもち，患者や配偶者に支援，治療できるようになってほしい． 今回，筆者は男性透析患者の性的問題とそれらに関連する精神医学的要因を図に示した．また，その治療やリハビリテーションについて記述した．性的問題には，生物学的要因，精神・社会的要因，精神医学的問題，夫婦の関係性などの多くの要因が複雑に作用しあっている．これらの問題の治療の原則はカウンセリング，行動療法，精神療法と血液循環の改善である．包括的に，しかし段階的に，治療とリハビリテーションを実行していくことが重要である．どんなに良い方法でも，患者や家族が実行できなければ意味がない．医療スタッフは治療法を強要してはならない． |

| [ Summary ] | Generally in Japan, sexual problems of patients with chronic diseases such as those requiring hemodialysis are neglected. Even when patients suffer from sexual problems, they do not actively seek help from their doctors. In addition, doctors do not ask patients about sexual problems. Even if patients seek help for sexual problems, there are few medical care professionals who can cope with patients' sexual problems. Medical staff need to take a great deal of interest in patients' sexual problems, and should try to provide all possible support to help patients and their wives. The author described sexual problems encountered by hemodialysis patients and many psychiatric factors related to male dialysis patients in a chart, and described some related treatments and forms of rehabilitation. Many factors, such as biology, psycho-social factors, neurotic factors, relations between husbands and wives, influence their sexual relations. Principle treatments are counseling, behavior therapy, psychotherapy and therapy to improve blood circulation. However, it is vitally important that we execute treatment and rehabilitation in a step-by-step fashion. If patients and their families cannot execute even the best treatment method, there is no meaning. The medical staffs must not force any treatment. |
