【 要旨 】 | 透析患者の合併症を少なくし,死亡率リスクファクターの改善をはかるためには,栄養評価を正確に行い,患者の栄養状態を良好に維持することが不可欠である.したがって,栄養評価は再現性が強く,臨床経過に即した指標を提供する必要がある.従来のキャリパー法による身体計測では,精度を高めるために測定の熟練を求められたが,生体インピーダンス法は簡便に測定でき,しかも再現性が高い.栄養評価による栄養状態は患者の透析量,すなわち透析療法の質とよく正相関する.したがって,透析量を適正に処方し,透析療法における患者のリスクファクターを軽減または改善するためにも,生体インピーダンス法は不可欠と思われる. |

[ Summary ] | In order to reduce mortality and morbidity in dialysis patients, it is very important to prevent protein energy malnutrition (PEM). Assessment of PEM severity and response to therapy requires measurement of nutritional status. Standard anthropometric measurements are carried out by dietitians. But, errors in measuring arm muscle circumstance are made by dietitians and degree of edema. Therefore, valid result in nutritional parameters are indispensable for the precise evaluation of nutritional status. Recently, the use of bioelectrical impedance analysis (BIA), InBody 3.0TM is gaining acceptance as one of body composition analyzer. This method is simple and non-invasive. Moreover, there are a few measurement errors as a result of using multi-frequency BIA. The following parameters related to nutritional status can be estimated; total body fluid, intra- and extra- cellular fluid, tissue edema and lean body mass, total muscle mass and mid arm circumstance. etc. Nutritional status is correlated directly with dialysis dosage. The accumulation of uremic toxins decreases appetite, and excessive body fluid increases inflammatory cytokines. Appetiteloss and increased cytokines promote PEM. Therefore, the measurement of nutritional status is indispensable to reduce mortality and morbidity in dialysis therapy. |
