【 要旨 】 一般的に，上肢血管の動脈硬化による症状発現は少ないとされているが，透析症例では，慢性腎不全による動脈硬化の進展に加えて，上肢に内シャントを作製するため，上肢に動脈硬化に基づく虚血性変化がみられることが多い．前腕動脈のメンケベルグ型動脈硬化症による内シャント作製困難な症例や，シャントトラブルによる前腕の動脈閉塞に手指の細小動脈硬化症をも加わった，内シャント作製後のスティール症候群にしばしば遭遇する．本稿では，これらの症例に対する外科的対策について述べる．

[ Summary ] Arteriosclerosis in the upper extremiies fails to produce as devastating lesions as in the lower extremiies in non-ESRD patients. In ESRD patients, however, far advanced arterio sclerosis with arterial calcification in the upper extremities makes it difficult to create an arteriovenous fistula and results in access-associated steal syndrome. Surgical management of these complications will be mentioned in this article.
