| 【 要旨 】 | 外来通院慢性腎不全患者治療におけるクリティカルパス(CP)の有用性について検討を加えた．対象患者10名の原疾患は慢性腎炎9名，糖尿病1名である．対象患者に対し，腎不全，腎機能，低蛋白食などの指導をCPにより行った．経過中に，バリアンス(変動，逸脱)として，脳梗塞，心不全を経験し，この2例は脱落症例となった．CPは患者指導時間の有効利用，インフォームド・コンセント，指導に対する安心感，にそれぞれ有効であった．また同一期間中に，低蛋白指導を行っていた慢性腎不全患者をコントロール群として比較したところ，CP群が低蛋白食実施のコンプライアンス向上に役立ち，加えて，多職種間の連携に有用であった．これらの結果より，CPは慢性腎不全患者の教育と治療に有用な手段と判断した． | |

| [ Summary ] | Our trial was planed to clarify the usefullness of the critical path (CP) in treating 10 outpatients with chronic renal failure. Their original deseases consisted of 9 with CGN and 1 with DM-nephropathy. We instructed them in treatment of chronic renal failure, renal functions, and the importance of dietary protein restriction, according to the critical path method. We experienced 2 variances, cerebral infarction and heart failure. These 2 patients dropped out in this study. CP is very useful for developing cooperation between doctors and co-medicals, to guidance for them and prevent wasting time, providing informed consent for and comfort patients. During the same period, we selected 10 other patients, receiving low protein diets, as a control group. The CP group exceed the control group in dietary protein restriction. Our team work has been strengthened by using the critical path method and we were able effectively conduct this study. We concluded that the critical path is a useful and effective method for the education and therapy of patients with chronic renal failure. |
