【 要旨 】 透析導入を余儀なくされた保存期の患者は，過去の経験の有無にかかわらず，予期的心配が働く．そこで今回，慢性腎不全患者に対して，予期的心配を最小限にとどめ，不必要な不安を与えることなく，透析導入がスムーズにいく方法を検討した．その結果，クリニカルパスを作成し，保存期から計画的にこれを利用することにより，これからの治療やケアについてのインフォームド・コンセントを得ることができた．さらに，患者・家族を含めたチームの協力体制が強化し，ケアの質の保証や一貫した継続教育に有効であった．

[ Summary ] Patients in the end stage of renal dysfunction worry about their health problems. We have produced a critical path (care program), in order to decrease patients' unnecessary anxiety and gradually introduce them to hemodialysis. The treatment schedule starts with outpatient clinical care, therefore patients may easily understand the total concept of clinical care, including nursing care. This program is also expected to reduce hospitalization period.
