【 要旨 】 |
各種アフェレーシス療法において,血漿交換に伴う血中アルブミンの喪失やデバイス内充填液による血液希釈の影響によって,膠質浸透圧は低下し循環血液量の減少が生じる.循環血液量の過度の減少は,血圧低下を招き,治療の継続を困難にする.本稿では,おもにアルブミンと循環血液量の関係に着目し,kinetic modelingを用い循環血液量の減少を予測し,適正な置換液量やアルブミン濃度の設定法および血液製剤によるプライミングの必要性などについて言及する. |

[ Summary ] |
In several types of aphereis therapy, the patient's blood volume often decreases over time during treatment, due to a drop in colloidal osmolarity. This is induced by blood dilution from the aqueous solution primer in the device before the treatment and albumin loss from treatment, especially in the partially discarded mode of the double filtration plasmapheresis. Kinetic modeling for albumin transport during DFPP treatment was introduced to explain changes in blood volume during treatment, in order to determine the optimal concentration of substitution fluid for treatment. |
