| 【 要旨 】 | 介護度3に認定された65歳以上の透析患者が，介護老人福祉施設・ケアハウス・介護老人保健施設に入所すると介護保険の適用となり，通院する透析施設では医療保険を請求する．介護力強化病院ではすべてを介護保険で請求することになり，介護療養型病床に入院すると介護保険で入院費用を，透析治療費は医療保険の給付の対象となる．医療療養型病床では，すべて医療保険の給付を受ける． 通院透析を担う医療施設では，通常の在宅で請求する場合，「入院外」で請求する場合と医療保険に請求する場合が発生する．介護療養型病床群の病院と診療所，介護力強化病院および介護老人保健施設での報酬を試算した．また，長期入院を想定しての医療保険の1日当り入院基本料を示した．介護・医療保険ともに，人員配置と設備基準に応じた入院基本料の設定がされている． |

This report provides a simulation of costs in sanatorium type medical care facilities for elderly, health services facilities for the elderly and hospitals for the elderly. The costs are 818,098 YEN, 747,575 YEN and 782,030 YEN, respectively. | |
