| 【 要旨 】 | daily home hemodialysisでは，頻回の血管穿刺により，アクセス血管の血栓，狭窄，さらには閉塞の頻度が増加するのではないかとの疑問が生じる．この点に関して，透析ごとに穿刺部を変更するのではなく，常にアクセス血管の同一部位を穿刺するbuttonhole穿刺を採用すれば合併症がより少ないとの報告がある．ところが，buttonhole穿刺のための固定した穿刺ルートを得るためには，同一の熟練した医療スタッフが1～2カ月間にわたってブラッドアクセスの穿刺を担当し，透析ごとに繰り返し同一部位を穿刺することが必須である． われわれは，透析終了時にポリカーボネート製スティックをアクセス血管内に留置し，次の透析の開始時にこれを抜去することにより固定穿刺ルートを作製する時間節約型固定穿刺ルート作製法を開発した．いったん，この方法により固定穿刺ルートを作製すると，その後は，透析ごとにこの固定穿刺ルートに沿って，先端が通常の穿刺針よりも鈍い，特別に作製した金属穿刺針を挿入することにより体外循環血流が得られる． |

| [ Summary ] | In daily home hemodialysis, there is concern that with frequent different site punctures there may be an increasing incidence of thrombosis, stenosis or obstruction. It has been reported that complications are fewer when the constant site puncture method (i.e., buttonhole method) is used to puncture the blood access vessel for treatment with repeated homedialysis, as compaired to when the different-dite puncture method is employed. For successful in when using the buttonhole method, it may be crucial that during the initial period the same experienced medical staff member punctures the access vessel each time. However, this may not be a realistic goal in most dialysis facilities. Therefore, we developed a timesaving method to create a fixed channel for buttonhole punctures. In this method, at the end of each hemodialysis session, we replace the plastic cannula by a stick, then remove it at the start of the next hemodialysis session. By using this method, a fixed channel is created, through which a dull needle is inserted by using the buttonhole method. |
