| 【 要旨 】 | 長期透析患者，高齢透析患者の増加に伴い，頻回にブラッドアクセストラブルを繰り返し，新たなアクセスの確保に難渋する症例が増えてきている．近年それに対し，多くの施設から長期留置用カフ付き皮下トンネル型カテーテル使用が報告されている．長期留置を達成するには抗血栓性，生体適合性，抗菌性，耐久性に優れていること，高い透析効率が得られることなどが要求される．皮下トンネル，カフの存在は自然抜去防止，出口部からの細菌の侵入防止に有効で，柔軟な材質はより深部の中心静脈へのカテーテル留置を可能とした．しかし，感染や血栓形成が未だ重要な合併症であることに変わりはない． 今後，長期留置用のカテーテルの重要性が高くなることが予想される．カテーテルの材質，形状，管理方法，合併症対策などについてさらに検討を重ねる必要がある． |

| [ Summary ] | In recent years tunneled, cuffed catheters have been developed for use as vascular access devices. These devices are particularly useful in patients where other vascular access sites have been exhausted or who have severe cardiovascular disease. The use of tunnel and a bonded cuff serve to anchor the dialysis catheter in the tunnel, and presumably, prevent migration of bacteria down the outer surface of the catheter. The soft material allows the placement of the catheter with its tip in the right atrium. Complications, however, both in terms of anatomic, thrombotic, and infectious issues, have emerged as a major problem with their continued use. It is expected that the long-term catheter will attain important place in the future of this treatment. The material, the shape and the style of management of catheters should be studied further. |
