| 【 要旨 】 | 本稿では，2000年4月に施行された介護保険について，その特徴と問題点を検討した．特徴としては，介護サービスが有料であること，保険給付に限界があること，それと利用者の選択が可能になったことがある．同時に，これらは介護保険の問題点も生み出した．それは，第一に保険給付の鍵である要介護認定システムが不透明なこと，第二に利用手続きに時間がかかること，第三に利用者が十分な情報を知らないことである． したがって，ソーシャルワーカーは，介護保険の影響をモニタリングしながら，透析患者に大きな不利益が生じないようにアドポカシーを行ったり，効果的に介護サービスが利用できるよう適切な介入をすることが必要だと思われる． |

| [ Summary ] | This article discusses the characters of and problems with the Long-Term Care Insurance Law which started in April, 2000. The characteristics discussed are cost of caregiving services, limitations of insurance payment, and user's choice of services. However, these characteristics raise new problems concerning insurance : first, the proceedure is not open todisclosure; secondly, it takes a long time until patients and their families can utilize caregiving services provided by insurance; thirdly, patients and their families do not fully possess the necessary information about insurance. Therefore, social workers must monitor how insurance influences the life of patients and their families, and become their advacates, while effectively intervening to utilize caregiving services. |
