【 要旨 】 透析と腎移植は慢性腎不全医療における車の両輪としばしば例えられる．しかし，透析担当医が果たして十分にその役割を果たしているだろうか? 透析導入に際し，生体腎移植，献腎移植について，患者および家族に腎移植のメリット，デメリットについて詳細に説明し，献腎移植についてはその手続きを行うことが求められる．また，生着後の管理についてもその役割を担うべきである．臨床腎臓病学の教育課程で腎移植患者を担当する機会を必ずもたせることが必須の事項である．

[ Summary ] Kidney transplantation is the ideal treatment option for end-stage renal failure. Only through this modality may a uremic patient be cured. No matter how much successful dialysis is delivered to a patient, patients still remain in chronic renal failure. Nephrologists who are responsible for chronic dialysis are required to select candidates, evaluate co-morbidity, maintain their condition to tolerate immunosuppressive therapy by means of adequate dialysis and submit the latest information to the transplant team on their patients' condition. Not only transplant surgeons but also nephrologist play an extremely important role in kidney transplantation.
