【 要旨 】 | 末期腎不全の治療は腎移植,血液透析,腹膜透析の3本の柱で成り立っている.患者,家族,医療スタッフは,それぞれの治療法の長所および短所を熟知し,そのうえで患者や家族の状況や希望により治療法を選択する.また,どの治療法も完全でないため,各々の治療法が順調に施行できない場合は,他の治療法に移行して治療を継続しなければならない.乳幼児においてはおもにCAPDによる治療法が選択され,年長児になると血液透析とCAPDとのどちらかを選択できる.そして移植を最終目標にしながらも現在の透析,とくにCAPDによるQOLの改善によりある程度の満足を得て,現在の透析療法を継続しているのが現状である.透析療法と移植を比較してみると,全体的には移植のほうが透析より優っていると思われ,移植を治療の最終目標として末期腎不全の患者の治療に当たるべきであると思われる.とくにCAPDにおける硬化性被嚢性腹膜炎の合併により,小児における腎移植の重要性が再認識されるようになってきている. |

[ Summary ] | Children with end-stage renal disease (ESRD) are treated either with renal transplantation (Tx), hemodialysis(HD)or continuous ambulatory peritonealdialysis (CAPD). CAPD is preferred for small infants, but older children can be treated with HD or CAPD. Renal Tx is believed to be the best treatment among these for growing children with ESRD. However, adequate treatment with CAPD by withholding early renal Tx and long-term CAPD treatment is a common practice for Japanese children with ESRD. The current recognition of sclerosing encapsulating peritonitis, as one of the major complications of long-term CAPD has prompted us to perform earlier renal Tx in Japan. |
