| 【 要旨 】 | 皮膚疾患に対するアフェレシスは，主として自己免疫性水疱性疾患に臨床応用されている．とくに尋常性天疱瘡や水疱性類天疱瘡に対して有用性が認められている．病因はそれぞれ抗表皮細胞膜抗体および抗基底膜抗体が考えられており，IgGクラスが主であり，この抗体の除去にアフェレシスが施行されている．アフェレシスには単純血漿交換法，二重膜濾過血漿交換法および血漿吸着法があり，種々の二次膜による分画除去やトリプトファンやデキストラン硫酸をリガンドとした吸着が施行されている．ステロイド薬や免疫抑制薬の使用が困難な症例や，症状が著しい症例や各種治療に抵抗性を示す症例に適応が考えられている． | |

| [ Summary ] | The plasmapheresis is an effective treatment for autoimmune bullous diseases of the skin such as pemphigus vulgaris and bullous pemphigoid. The pathogenesis of such skin diseases are autoantibodies directed against the intercellular cement surface and the basement membrane zone respectively. As these autoantibodies are mainly IgG class of immunoglobulin, the plasmapheresis is used for a removal of such circulating autoanti-bodies from the blood. The methods of plasmapheresis are a single plasma exchange, a double membrane filtration plasma exchange using separation by second filter and an immunoadsorption using a ligand of tryptophane and dextran sulfate. The indication for plasmapheresis in autoimmune skin disease should be in the case of drug resistance, difficulty of using drugs and severe skin lesion. |
